What to bring to the agency
- New Jersey driver license/ID.
- A $15 fee to be paid by credit/debit card, cash, check, or money order (payable to NJMVC).
- You may bring a completed Application for Driver History Abstract or fill it out at the agency.
What you need to do at the agency
Complete the Application for Driver History Abstract (Form DO-21) and submit it with the $15 fee at one of the counters.

Complete Application for Driver History Abstract (Form DO-21). In Section B, select the document type(s) and date for each, with the reason for your request.
Mail the Application with a separate check or money order (payable to NJMVC) for each document and a photocopy of your New Jersey driver license/ID to:
